Signing up for FollowMyHealth®

Sign up just got easier! If you are a patient 18 and older you will receive an auto invitation, this will come to the email address that you have provided to us. When you check in for your appointment, please verify or provide your email address to the reception staff at check in time.

If you do not have an upcoming appointment, and would like to sign up for FollowMyHealth® you can stop by any Hutchinson Clinic facility and see a receptionist at your convenience to enroll. To help protect your privacy and identity, we require patients to present a valid photo ID and supply a current email address in person and do not allow requests in any other way. This ensures patient’s records are loaded to the appropriate individual.

After you have received the email invitation, you are ready to register and start enjoying the benefits of FollowMyHealth®. Follow these steps to help you through the registration process.

Authorized Individual Access

Sign up is available for minor children and dependent adult patients. There are specific forms that need to be filled out for these patients. Please visit the Authorized Individual(Proxy) page to learn more.
